current referee reports x 2
evidence of vaccination preventable diseases (VPD)
correctly certified ID
AHPRA registration with no restrictions
CV no older than 12 months
references within 12 months and from last place of employment
up to date ALS

EMST as applicable
certified copies of identification and qualifications required
