- FRACGP

- Up to date ALS
- PAL (APLS)
- EMST etc as applicable
- Minimum requirements: Up to date and comprehensive CV with gaps of three months or more explained
- Current referee reports x 2 (current placement and within 12 months)
- Evidence of vaccine preventable diseases (VPD)
- Correctly certified ID
- Evidence of flu vaccination
- AHPRA registration with no restrictions
- Criteria for acceptable CV and referees: CV – no older than 12 months
- must include most recent position held. Gaps of 3 months or more must be explained. References – 1 within 12 months

Candidate information will not be forwarded to the Facility Manager without meeting the above criteria. - Mumps
- Rubella (MMR): Minimum one dose
- second dose within three months of commencement. Varicella (Chicken Pox): Minimum one dose
- second dose (if required) within three months of commencement. Hepatitis B: Minimum two doses
- third dose within six months of commencement. Pertussis (Whooping Cough): One dose
