HITH Position
The primary purpose of the position is to provide Hospital in the Home (HITH) patients with safe, appropriate, patient focused, outcome directed, coordinated quality care that meets professional standards and is based on best practice standards.. Provide high-quality, patient-centred medical care to Hospital in the Home (HITH) patients under supervision, including comprehensive assessment, care planning and delivery in line with professional and best-practice standards.
Communicate clearly and effectively with patients, families/carers and the multidisciplinary team to support coordinated care, timely escalation of clinical issues, and continuity of treatment, while maintaining confidentiality and privacy.
Ensure accurate, timely and compliant clinical documentation, including medical records, referrals, discharge summaries, reports, and other required patient-related documentation.
Contribute to the education and development of clinical staff through on-the-job teaching, case discussions and feedback.
