About this position
Locum candidates must be prepared to participate in 100% of the roster, including all shift types and on-call requirements. Required documentation: CV (chronological format, MM/YYYY – MM/YYYY, gaps of 6 weeks or more explained), 2 references (from clinical supervisors who have observed the locum within the last 12 months), applicant declaration (wet signature), VPD form and TB Questionnaire (including immunisation evidence and updated flu vaccination), photo ID (current and in-date passport or driver’s licence in colour), immigration notification and VEVO (if overseas passport, with agency as employer), VEVO permission form, qualifications (base degree, fellowship, GESA), CPD activity statement (Specialist only), criminal history consent form (noting updates to B45 policy, ALL locum placements require a Criminal History Check regardless of timeframe of placement or Current National Police Clearance), confirmation of Medicare Specialist recognition for billing purposes (Medicare letter or PRODA screenshot with date, time stamp, and Medicare Australia header). Candidates who have separated from Queensland Health within 3 months of the commencement date of a locum placement will not be considered by our HHS. Contracts are not generated for placements under 7 days.
